Lena Dziedzic is a 55-year-old female presenting to the clinic for management of hypothyroidism. She reports ongoing symptoms, including fatigue, weight gain, and sensitivity to cold, which have persisted despite being on levothyroxine 100 mcg daily. Lena’s TSH levels at her last visit were elevated at 7.2 mIU/L, indicating suboptimal control. She has a history of primary hypothyroidism diagnosed five years ago and struggles with medication adherence, as she sometimes forgets to take her levothyroxine on an empty stomach.
